Requirements for Applying Online:
- Time to complete the application in ONE sitting (approximately 15-20 minutes)
- At least one email address for you or a family member
- Ability to print from the computer on which you are applying online
The General Guidelines webpage and ensembles webpages contain pertinant information, including audition information. Be sure to read these BEFORE beginning this online web application.
Once you have completed this application: All applicants will need to print out a copy of the Application Confirmation, sign it as the applicant, have a parent/guardian sign it, and mail it to the Michigan Youth Ensembles Office. A $45 application fee is required. You can pay
by credit card at the bottom of the application form or mail a check or money order with your Application Confirmation. Only complete applications will be reviewed.
Application Deadlines:
Application deadline is March 31, 2009. Application and fee must be received by this date. Applicant will then be emailed specific audition information the week of April 13.
IMPORTANT: You will not be able to save your work on the online application and return to it at a later time so gather the following information before beginning the online application:

- Mother’s AND Father’s (or Guardian) Names

- Home addresses, if different from yours
School Information:
- School Name
- Complete Street Address
- City, State, Zip Code
Private Music Teacher:
- Name
- Phone Number
- Years of study with this teacher
- Is teacher affiliated with UM-School of Music, Theatre & Dance?
Credit Card Information if paying $45 application fee by credit card (Visa, MasterCard, Discover)

- Name as it appears on card
- Expiration Date
