The application requires signatures from both the applicant and a parent/guardian agreeing to "abide by all MPulse Ann Arbor Standards and Policies while a member of the program".   That document is made available here for your information during the application process.   Admitted students will receive a copy of this document in post-admission materials.

The following PDF documents are posted for use by students admitted to MPulse Ann Arbor for Summer 2008. Copies of all of these documents are included in admission packets.  However, they are also posted here should a form be misplaced.  Please note that tuition payments are due within 30 days of the invoice date.  All of the forms listed below are due before May 30.
