School of Music, Theatre & Dance Application Materials Checklist

These elements should be sent directly to the School of Music, Theatre & Dance:

• Recommendation from a teacher in an appropriate arts area
  
• Resume and repertoire list uploaded with online application or mailed to the School of Music, Theatre & Dance
  
• Portfolio (if applicable – see descriptions for Composition, Performing Arts Technology, or Theatre Design & Production)
  
• Writing sample (Bachelor of Theatre Arts, Musicology, or Music Theory)
  
• Completed Request for Audition/Interview form (only if using the paper application)

In addition to the completed application, these elements should be sent directly to the Office of Undergraduate Admissions:

• Test scores (SAT or ACT)
  
• Transcripts
  
• High school counselor form
  
• Academic teacher recommendation
