Undergraduate Application Materials Checklist

The School of Music, Theatre & Dance requires two application components: the Common Application (or the Cross-Campus Transfer Application) and a DecisionDesk profile. The requirements for each component are listed below.

DecisionDesk Profile

All SMTD applicants are required to create a DecisionDesk profile to demonstrate their artistic preparation. While completing your DecisionDesk profile, you will be required to submit:

  • A recorded audition may be uploaded for applicants living 300+ miles from Ann Arbor and unable to attend an in-person audition. Certain programs—including Dance, Musical Theatre, Percussion, Organ, Theatre & Drama—do not allow recorded auditions without the express consent of the faculty.

Online Application

In addition to submitting your DecisionDesk profile, you must submit the online application and accompanying credentials by our deadline in order for your application to be considered complete.

Freshman Applicants

All Undergraduate applicants are required to complete the Common Application, including the U-M Writing Supplement. In addition, please submit the following required documents:

  • Your high school transcript—If you have taken courses at a non-U.S. high school, an official English translation is required to accompany the non-English language transcript.
  • A school report—The School Report must be returned with your official high school transcript and be received in our office by our deadline. We strongly encourage high school counselors to fill out this form via the Common Application and submit the transcript electronically.
  • One academic teacher evaluation
  • SAT or ACT (with writing test) scores—Your SAT/ACT test scores must be sent directly from the testing agency and be received in our office by our deadline. Please plan ahead—rush paper scores are not accepted. If you have not yet reported your scores to the University of Michigan, you may request them to be sent online:
    ACT (U-M code is 2062)
    SAT (U-M code is 1839)
  • TOEFL, MELAB, or IELTS scores (for non-native speakers of English)

Transfer Applicants

All transfer applicants must complete the Common Application, including the U-M Writing Supplement. In addition, please submit the following:

  • A final high school transcript showing diploma earned and graduation date.
  • An academic teacher recommendation
  • Registrar's Report—The University of Michigan does not require the registrar's report for transfer applicants. However, the Common Application will request the name of your college/university registrar or appropriate dean/college official who can verify your enrollment and academic standing.
  • College transcripts showing any college coursework in progress and/or all college coursework completed

In addition, please visit our Office of Undergraduate Admissions for information about transfer credit.
