Forms
- Recital Approval Jury (.pdf or .doc) - needed for recital approval juries, this form has to be signed by all three faculty members in the jury and submitted directly to the Chair by the student's teacher.
- Departmental Recital Request (.pdf or .doc) - needed in order to perform in a String Departmental Recital, this form has to be filled in detail by the student, and then signed by the student's teacher. If this is for a chamber group, the form has to be signed by the groups coach.
